Huddle Donation Programme Setup Form

Before we pass your donation request on to Huddle for approval and fulfilment, we will require you to complete Huddle’s Setup Form.

Please note: We will send you the form, including submission instructions, as soon as you have placed your donation request on-line.

You will be required to provide the following information:

• CTX Order Number
• Charity Name
• Web address of Charity
• Name and role of contact for this application process
• Telephone number of contact
• Registered address of Charity
• Name and role of the person that will manage Huddle in your organisation (Huddle will set them up as the first user and give them admin permissions)
• Email address of Huddle Manager
• Telephone number of Huddle Manager
• How do you intend to use Huddle in your organisation?
• Confirmation that Huddle can use your Charity as a case study for future Huddle marketing campaigns (this will always be done with your full approval)
